The Pennsylvania Department of Health is conducting its annual questionnaire of long-term care facilities for the data reporting period of 7/1/2025 through 6/30/2026. This web-based questionnaire begins September 1 st and must be completed by your staff and submitted to the Department electronically by Friday, October 23, 2026.
Submissions may be made at any time, but we recommend against waiting until the last minute to submit. A reminder that completion of this questionnaire is mandatory. If you do not complete the questionnaire, your facility could be issued a deficiency and a fine.
After completion, if there are still any outstanding issues, DOH will reach out to you to request that you provide an adequate explanation for the issues at hand or ask that you revisit your data and correct any potentially inaccurate data.
